Intent to Register Form
North Middlesex Minor Hockey has opened the intent to register for the 2021/22 season. This form will provide the executive to plan for the upcoming season, there are many variables that goes into planning a season and we want to be prepared as much as possible for the 2021/22 in hoping we will have a true hockey season. As of right now there will be no funds collected. Please complete the following form:
