Covid Vaccination Policy
Submitted By Nicole Dare on Tue 09 21, 21
On behalf of North Middlesex Minor Hockey, thank you for registering your child for the 2021-22 hockey season. We’re looking forward to seeing the kids back on the ice and playing meaningful games again. We appreciate how challenging a time this is for all of you as we continue to deal with the impact of COVID-19 as we return to hockey.

NMDMHA is mandated by OMHA and therefore we must follow the guidelines and policies when it comes to the return to hockey. As of now the current policy states:

“The OMHA requires that all Affected Persons be Fully Vaccinated against COVID-19, in accordance with the remainder of this policy, as a condition of participating in any Sanctioned Activities during the 2021-22 hockey season. At present, this policy does not apply to hockey players or spectators born after 2009.”

All Affected Persons are required to by fully vaccinated by 18 October 2021

OMHA will comply with Ontario Human Rights code and provide accommodation to any affected person who is unable to be vaccinated for substantiated medical reason and/or on such other grounds under the OHRC, unless such accommodation cannot be made without inflicting undue hardship on other affected persons

To begin the accommodation request process, please complete the application by visiting the following link: https://form.jotform.com/212446027965055

*For complete OMHA Vaccination Policy please see attached document.

Furthermore because we rent the ice from the Municipality, we must follow their guidelines when it comes to entry to any North Middlesex Municipality facilities;

Anyone 12 years of age and older NOT participating in ORGANIZED SPORT  entering an indoor recreational facility must be fully vaccinated from COVID-19 as of 22 SEPTEMBER 2021

Failing to comply with the requirements of O REG 364/20 can result in charges under the ROA. Set fine amounts are $750 for individuals and $1000 for corporations

Users should arrive to the facility 40 minutes before their scheduled ice time

All users will be greeted by a security guard who will be checking ID, proof of full vaccinations and ensure users scan the QR code upon entry

Any patrons who are abusive to the security guard ,staff members, volunteers or other patrons will be removed from the facility and given notice of a one week ban and if second occurrence take place the patron will receive a ban for 2021-22 hockey season

Patron with medical exemption are required to present identification and written documentation stating the individual is exempt for a medical reason to the business or organization

*Please read attached Municipality Of North Middlesex Arena COVID Safety Plan

Ontario Women’s Hockey Association (OWHA) is meeting on 22 September 2021 to confirm their COVID-19 policy.

I encourage everyone to read the 3 attached documents for a complete understanding of the COVID-19 return to play guidelines/policy.

If you have any further questions please don’t hesitate to email or call me and I’ll do my best to answer any questions you may have. If you have any questions regarding Municipality COVID safety plan you can contact Brandon Drew Manager, Recreation and Facilities at [email protected]

I understand and respect the above guidelines may not be in line with your family's opinions or values. However these are the guidelines we must follow to allow our kids to return to the game we all love. If you would like cancel your registration please do so by 1 October 2021 for a full refund.
