Vulnerable Sector Check Form (Police Record Check)
To help keep our players safe, it is required that all members of the bench staff for each team and all executive board members need to have a new Vulnerable Sector Check Form (Police Record Check) for season hockey season.

Your Vulnerable Sector Check Form must be submitted to our OHMA Rep, Kelly, by September 30, 2022.
