To help streamline cheque nights and ensure complete registrations, all NMDMHA players must fill out the NMDMHA 2025/2026 Player Information Form
This form is required for our internal association records and does not replace your official registration. One form is needed per player registered.
Please fill out the form for your player by July 29, 2025.

Important Dates:
• Returning OMHA players: $200 late fee applies after June 16, 2025
• Player Information Form deadline: July 29, 2025
• All cheques & full payments due: September 1, 2025
