North Middlesex Jr. Stars Registration

Thank you for registering with North Middlesex District Minor Hockey Association!

Please read ALL of the following information, as it pertains to your requirements for the upcoming hockey season.

 Important Dates
 OWHA (Girls) Players

Registration will remain open until June 15, 2024 for all new OWHA players.

Registration Deadline for NMDMHA returning players was May 5, 2024.

After May 5, 2024 there is a $200 Late Fee for RETURNING players

 OMHA Players

Registration for NMDMHA returning players is open
from May 15, 2024 to June 15, 2024.

After June 15th, 2024 there is a $200 Late Fee for RETURNING players.

Items Needed for the 2024/25 Season 

 New players to NMDMHA


· A valid copy of your child(ren)’s Birth Certificate must be uploaded to their Spordle account.

· At least one parent must complete the parent/guardian version of Respect in Sport (click here for course)
          o Respect in Sport only has to be completed one time

· Submit all registration fees and post-dated cheques (listed below) on the designated cheque collection night

· Complete and submit both the player and guardian contracts and medical information to your teams head coach at the start of the season (click here for forms)

· A copy of your player's parent's current address in the form of a tax bill or driver's license, including the date, must be uploaded to their Spordle account.

ALL
NMDMHA players
 

· Submit all registration fees and post-dated cheques (listed below) on the designated cheque collection night

· Complete and submit both the player and guardian contracts and medical information to your teams head coach at the start of the season (click here for forms)

· Every family must have at least ONE parent/guardian complete the parent/guardian version of Respect in Sport course (click here for course)

Please note:
o The Parent version is different from the Coaches version
o This course only has to be completed one time
o You must make sure to link the course to ALL children on your profile
o If you have already completed the course, make sure to add any new players to your profile

· A copy of your player's parent's current address in the form of a tax bill or driver's license, including the date, must be uploaded to their Spordle account.

·

IMPORTANT:

Please note that your child(ren)’s registration will not be considered complete,
and they will subsequently be unable to go on the ice until the following have been completed:

· All payments have been received, including all post-dated cheques (listed below)

· Avalid copy of your child(ren)’s Birth Certificate must be uploaded to their Spordle account

· At least ONE parent/guardian has completed the parent/guardian version of the Respect in Sport course

· Both the Player and Guardian contracts and medical information has been filled out
and returned to your team's head coach at the start of the season (click here for form)

• Your player's Spordle account is completed, including having up to date proof of address


Cheque Collection Night

Below is a list of the post-dated cheques required to be submitted prior to your child(ren) taking the ice.

All cheques must be filled out correctly for each player with proper dates, their name and age group in memo line and amounts prior to drop off. If your paperwork is not completed correctly you will be asked to do so before handing in anything. Each player MUST have their own set of cheques, siblings cannot be grouped together on any payments or cheques.


There will be 2 designated times and dates to drop off post-dated cheques, and pick up cash calendars for the season.
Tuesday, August 6 from 5-8pm or Tuesday, August 13 from 5-8pm

***These will be the ONLY opportunity to get your cheques in. If you are unable to make this time,
we kindly ask that you arrange for someone to drop your player(s) cheques off for you on this night.
It is YOUR responsibility to ensure all payments and post-date cheques are in on time.***


 
Fees/Post-Dated Cheques

The following is an outline of the fees required for ALL NMDMHA players (new and returning).

 

Registration
by Cheque (payable to NMDMHA), Cash, Money Order, or E Transfer
Note: E-transfers will ONLY be accepted after July 1st, 2024.
E-transfers can be sent to [email protected]
Each cheque and EMT must have your players FULL NAME and AGE DIVISION in the memo line.

U5 – $200.00

U7 – $275.00

U9 – $425.00

U11-U18 - $700.00

Players New to NMDMHA U9, U11, U13, U15 & U18
$200.00

After June 18th, 2024 there is a $200 Late Fee for RETURNING players.


***ALL returning players can earn a $25 credit towards their registration fees for every new player you recruit to NMDMHA.
Credits earned may only be applied to a registration fee for U9-U18 for OMHA & WOGHL returning players***

 

Post-Dated Cheques

The following are the post-dated cheques you must provide and date properly for all players for the upcoming season.
All cheque MUST have your players FULL NAME and AGE DIVISION in the memo line and correct date to be accepted.


Each player must have their own set of cheques provided with their name and age division included in the memo.
You CANNOT add cheques together for your players.
For example: If you have 3 players, you hand do 3 separate $300 post-dated volunteer cheques, one for each our your players.

Cash Calendar Cheque (5 cash calendars) – 1 cheque per family

o Payable by CHEQUE ONLY to NMDMHA

o Amount: $125.00

o Post-dated for: Dec 1, 2024

 

Rep Player Tryout Fee – 1 cheque per player trying out for a rep team in the U11, U13, U15 & U18 OMHA divisions.

o Payable by CHEQUE ONLY to NMDMHA

o Amount: $50.00

o Dated for: current date

Please note: This cheque will be cashed once your player participate in any of our U11, U13, U15 & U18 OMHA rep teams tryouts. This includes if you player is just trying out for the possibility to be AP'd or for the ice time.
Tryout cheques will be cashed even if your player does not make the rep team.

Sponsorship Commitment – 1 cheque per player (U6, U7, U9. U11, U13, U15 & U18 OMHA & WOGHL divisions)

Please choose ONE of the following two options (one option PER PLAYER)

o Donation – payable by cheque (to NMDMHA), cash or money order

o Amount: $100.00

o Cheque post-dated for: Dec 31, 2024

OR

o Additional Cash Calendars (4) – payable by cheque (to NMDMHA), cash or money order

o Amount: $100.00

o Cheque post-dated for: Dec 31, 2024

Jersey Deposit – 1 cheque per player (U9, U11, U13, U15 & U18 OMHA & WOGHL divisions)

o Payable by CHEQUE ONLY to NMDMHA

o Amount: $50.00

o Post-dated for: Apr 30, 2025 (to be given back upon return of jersey in good condition at the end of the season)

 

Annual General Meeting (AGM) – 1 cheque per family

o Payable by CHEQUE ONLY to NMDMHA

o Amount: $50.00

o Post-dated for: Apr 30, 2025

 

Volunteer Commitment – 1 cheque per player (U6, U7, U9. U11, U13, U15 & U18 OMHA & WOGHL divisions)

o Payable by CHEQUE ONLY to NMDMHA

o Amount: $300.00 per player
Each player must have their own cheque with their name and age division in the memo.
Do not put all your players on one cheque.

o Post-dated for: Feb 28, 2025

Please note: ALL families must submit volunteer cheques,
regardless of if a parent/guardian plans to volunteer as bench staff, participate on the Executive, etc.


Everyone family needs to please fill out and email the Volunteer Bond Policy & Agreement to
Lindsay at [email protected] before August 13, 2024. (click here for form)

There is an option to opt out of volunteering and just submit a cheque of $300.00 per player.

If you choose to work your volunteer credits, a total of 5 credits per player are needed to get your cheque back.
