Reorganization Inaugural Board Application Form
Each centre will be allocated 5 seats on the Inaugural Board that will be selected by the current respective Boards (North Middlesex, Lucan & Ilderton Minor Hockey Associations).

Each will follow an application and vetting process in order to select the successful candidates for the 5 seats in their own association. Please fill in the our application to be considered for a position on behalf of North Middlesex Minor Hockey Association. At this time, please complete the application form due by June 8th.
